ConnectMe is Think of Us' resource navigation platform that helps young people and kinship caregivers connect to the services, supports, and information they need, when they need them. It combines AI-powered intake and resource matching with trained Care Navigators (CNs) who provide personalized, one-on-one support throughout the process.
This platform is not a referral directory. Instead of simply providing a list of resources, CNs work alongside each person to navigate services, overcome barriers, and stay engaged until their needs are met. AI helps make support faster and more efficient, while people provide the guidance and connection that lead to real outcomes.
ConnectMe is not just a technology tool, it is a practice model. The platform handles intake, urgency coding, and resource matching at speed, while Care Navigators bring empathy, cultural awareness, and follow-through. Technology creates capacity. People create connection.
